Adult Social Care & Wellbeing drop in sessions – Welcoming Space Brackley Library
As part of the wrap around support development in our Welcoming Spaces we have great news in promoting the new drop-in sessions from Adult Social Care at Brackley Library starting from 17 May 2024 10am – 12pm.
The team will be able to assist people to connect in their community, access resources to achieve a sustainable and positive wellbeing and focus on people’s strengths and community assets. They support people who may be at risk of abuse to gain control and make them safe again (Safeguarding).
They also look at what a good life looks like if long term support is needed and identify the best outcome with the individual (Care Act Assessments) and work with them to identify and provide support (Support planning). Reviews are completed for people in the community in receipt of social care support.
